This Advanced Practice Provider provides patient care in accordance with allied health privileges granted by OHSU and consistent with the credentials and the philosophy of the Department of Medicine, University Hospitals and Clinics.   This position will allow for additional provider coverage in our Community Oncology clinics.   Functions and duties of the position include: - Address urgent needs, evaluate patient situation, and determine the tolerance of the treatment; - Review labs and modify orders as needed; - See new patients in conjunction with attending physician; - Write chemotherapy orders when treatment plan has been initiated by attending physician in compliance with credentialed privileges; - Manage patient anticoagulation, as appropriate; - Work closely with phone triage staff to manage patient issues and problems, and see patients emergently as add-on/walk-ins, as appropriate, or requested by attending physician; - Admit patients to community hospitals, as appropriate, in collaboration with attending physician; - Round on patients in community hospitals, as requested by attending physician, in compliance with community hospital credentialed privileges; - Function in a manner which maintains the confidentiality and privacy of each patient, promotes a positive relationship with all health team members and supports the policies and procedures of the University Hospitals and Clinics.

We are seeking a dynamic individual to join our team within IDD/CDRC. The Advanced Practice Provider (APP) provides collaborative and multidisciplinary care in the OHSU/CDRC Spina Bifida Program, Feeding and Swallowing Disorders Clinic, and the Autism Clinic with a team of attending physicians, APPs, and allied health professionals. The APP will provide diagnosis, consultation, and treatment (to include care coordination and referral services) to individuals and their families with spina bifida, complex feeding difficulties, and autism. For patients with Spina Bifida, the APP will provide specialty care, especially related to bowel and bladder functioning, family coping, self-care, self-concept, growth and development and consultation to other community agencies and providers. The APP will work within an interdisciplinary model, coordinate clinic activities, and have primary responsibility for case management and care coordination. The APP shall provide services as assigned by the supervisor in furtherance of the university’s missions and goals of teaching, research, patient care, outreach and public service.   The APP faculty will have opportunities to become involved in quality improvement work, education, research, and childhood advocacy. Doernbecher Children’s Hospital, the Department of Pediatrics, and the Division of IDD/CDRC are committed to developing our faculty members’ careers. A pathway for academic promotion for the APPs was recently implemented at OHSU.   Professional participation is expected including attendance in relevant educational conferences and didactic conferences. Clinical Activities (not related to teaching or research): This position provides primary clinical care in the Center for Women’s Health primary care clinic. This position works with a multidisciplinary team to provide comprehensive primary care to women, both adult and adolescent. This care may expand to male patients. This faculty member is expected to provide an outstanding level of clinical performance. The expectation is that clinical performance be delivered in a manner that is compliant with appropriate regulations and guidelines (JCAHO, Centers for Medicare and Medicaid, and all other state, federal, and institutional regulatory guidelines).   Scholarly Activities (research/writing, abstracts, grant supported work, presentations, etc.): Like all OHSU OB/Gyn faculty members, advanced practice providers are expected to have some scholarly activity, this may be limited to educating learners as outlined below, but may also include research, and other grant supported work or academic papers and presentations in disciplines related to their field of expertise. APP faculty members are often called upon to partner with other providers and members of the department or division research support staff in research activity. APP faculty members are expected to participate in clinical research as needed, often with the help of trained research support staff and senior faculty principal investigators.   Education (include types of teaching and types of student taught): This position provides clinical education to fellows, residents, and medical students participating in either OB/Gyn, Internal Medicine or Family Medicine rotations. As part of the multidisciplinary team charged with educating learners in the OB/Gyn or Primary Care environment, this person would be expected to provide feedback and evaluation, both formally and informally, of learners working in the CWH Primary Care Clinic. This person would also be expected to provide feedback and guidance in the continuing development and improvement of educational programs in OB/Gyn and possibly in primary care educational programs. The Program Director must administer and maintain an educational environment conducive to educating the residents in each of the ACGME competency areas. The Program Director must: - oversee and ensure the quality of didactic and clinical education in all sites that participate in the program; - approve a local director at each participating site who is accountable for resident education; - approve the selection of program faculty as appropriate; - evaluate program faculty and approve the continued participation of program faculty based on evaluation; - monitor resident supervision at all participating sites; - oversee development and execution of a comprehensive curriculum that meets the specialty requirements for training in that specialty, including overall goals and rotation-specific goals and objectives. - oversee development and execution of a competency-based evaluation system to monitor the progress of individual residents throughout the program. - continue implementation process of Entrustable Professional Activities (EPAs) program - ensure that faculty evaluates resident performance for each educational experience and documents the evaluation. - ensure that the program completes and documents a formal systematic evaluation of the program at least annually, specifically assessing the effectiveness of the program in meeting the overall goals of training in that specialty. Ensure that the attributes of the residency program are explicitly defined: resident performance, program quality, faculty development, graduate performance. - develop program-specific policies as discussed in the ACGME Institutional Requirements for selection, evaluation, and promotion of residents, disciplinary action, and supervision of residents. - prepare and submit all information required and requested by the ACGME, including but not limited to the program information forms and annual program resident updates to the ADS, and ensure that the information submitted is accurate and complete; - provide each resident with documented semiannual evaluation of performance with feedback; - ensure compliance with grievance and due process procedures as set forth in the Institutional Requirements and implemented by the sponsoring institution; - provide verification of residency education for all residents, including those who leave the program prior to completion; implement policies and procedures consistent with the institutional and program requirements for resident duty hours and the working environment, including moonlighting, and, to that end, must: - distribute these policies and procedures to the residents and faculty; - monitor resident clinical and education work hours, according to sponsoring institutional policies, with a frequency sufficient to ensure compliance with ACGME requirements; - adjust schedules as necessary to mitigate excessive service demands and/or fatigue; and, - if applicable, monitor the demands of at-home call and adjust schedules as necessary to mitigate excessive service demands and/or fatigue. - monitor the need for and ensure the provision of back up support systems when patient care responsibilities are unusually difficult or prolonged; create contingency plans for unexpected absences due to illness, inclement weather, bereavement, family leave, etc, and assist with communication to site directors, faculty, and trainees as needed. - comply with the sponsoring institution’s written policies and procedures, including those specified in the Institutional Requirements, for selection, evaluation and promotion of residents, disciplinary action, and supervision of residents; - be familiar with and comply with ACGME and Review Committee policies and procedures as outlined in the ACGME Manual of Policies and Procedures; - obtain review and approval of the sponsoring institution’s GMEC/DIO before submitting to the ACGME information or requests for the following: - all applications for ACGME accreditation of new programs; - changes in resident complement; - major changes in program structure or length of training; progress reports requested by the Review Committee; - responses to all proposed adverse actions; - requests for increases or any change to resident duty hours; - voluntary withdrawals of ACGME-accredited programs; - requests for appeal of an adverse action; - appeal presentations to a Board of Appeal or the ACGME; and, - proposals to ACGME for approval of innovative educational approaches. - obtain DIO review and co-signature on all program information forms, as well as any correspondence or document submitted to the ACGME that addresses: - program citations, and/or - request for changes in the program that would have significant impact, including financial, on the program or institution. - devote his or her principal effort to the program. - designate other well-qualified surgeons to assist in the supervision and education of the residents; - be responsible for all clinical assignments and input into the teaching staff appointments at all sites; - ensure that conferences should be scheduled to permit resident attendance on a regular basis, and resident time must be protected from interruption by routine clinical duties. Document of attendance by 75% of residents at the core conferences must be achieved; and, - ensure that the following types of conferences must exist within a program: - a course or a structured series of lectures that ensures education in the basic and clinical sciences fundamental to surgery, including technological advances that relate to surgery and the care of patients with surgical diseases as well as education in critical thinking, design of experiments and evaluation of data; - regular organized clinical teaching, such as grand rounds, ward rounds, and clinical conferences; - a weekly morbidity and mortality or quality improvement conference. - along with the physician faculty, assess the technical competence of each resident. - ensure that each resident has met all requirements for American Board of Surgery eligibility.

This position is responsible for the evaluation of patients (pediatric and adult) for general audiology, implants (BAHA and CI) and Hearing Aids. This position will counsel patients and their families regarding device options, use, care, maintenance, and realistic expectations. In addition, this position provides clinical audiology support. This may include, but not be limited to pure tone audiometry, speech audiometry, electrophysiological assessments, including ABR and OAE.   In addition, this position will provide education and training to medical students, residents and audiology fellows in a clinical setting through assigned lectures.   Participation in outreach programs directed to health professionals, patients, families, and the general public. In addition, will provide rehabilitation services.

The Advanced Practice Provider (APP) for the Department of Neurological Surgery possesses specialized knowledge and skills in the care of adult patients with neurological disorders. Overall responsibilities include inpatient and outpatient care and clinical support operations for the Department of Neurological Surgery. Works in partnership with faculty physicians to provide excellent clinical care and customer services in furthering OHSU mission of excellence in education, research and scholarship, clinical practice and community service. This position will primarily serve neurosurgery patients at the OHSU Health Hillsboro Hospital site (formerly Tuality Healthcare).   Works in partnership with faculty APPs and physicians to provide excellent clinical care and customer services in furthering OHSU mission of excellence in education, research and scholarship, clinical practice and community service. Provides clinical modeling and education to departmental staff, house staff, and nursing staff.    Inpatient duties: In the operative and inpatient setting, patient care duties include: Assists with neurosurgical procedures, including craniotomies, spinal procedures (including instrumentations) and microscopic procedures. Conducts hospital rounds daily on all patients, including intensive care units. Writes orders and progress notes. Performs all admission history and physicals and inpatient consultations. Orders appropriate laboratory and radiographic tests, such as MRIs, CT scans, myelograms, bone scans and other radiographic studies.   Outpatient duties: In the outpatient setting, patient care duties include evaluation of both new patients and follow-ups and the completion of pre-operative assessments; triaging calls, prescription refill management, and care coordination.   This position is primarily Monday to Friday, with scheduled on-call shifts and weekend coverage from home or in the hospital.

The appointee will be expected to plan, develop, organize, and direct a fully funded and independent research program that incorporates the use of non-human primate models. Incumbents are expected to independently and effectively direct a research laboratory, seek opportunities for internal and external collaborative research studies, supervise research staff and trainees, publish regularly in peer-reviewed scientific journals, and present/participate in national or international meetings. Teaching obligations include lecturing as well as mentoring/supervising graduate and/or postgraduate trainees.
