The OHSU Division of General Medicine & Geriatrics has built an integrated team practice model for delivering high quality medical care and service to the patients we serve. Function/Duties of Position

The APP (Advanced Practice Provider) will work collaboratively with the Skilled and Long-Term Care Geriatrics team and Division of General Internal Medicine & Geriatrics to further development, implementation, operation, and evaluation of the interprofessional inpatient geriatric program to improve the care we offer to older adults in post-acute and long-term care.

 

Duties include but are not limited to:

  • Provide direct patient care at Mirabella and Holladay Park Plaza skilled nursing facilities, 5 half days/week (plus a float day every other week). Your schedule may vary week to week depending on the needs of the facilities and coverage for providers who are out is expected, but should not exceed average expected sessions. After-hours coverage for the the Skilled and Long-term care team is included in your duties, averaging one week out of seven (weeknights and weekend call) and includes a minimum of one holiday per year.
  • Participate in care transitions and coordinating SNF workflow around bundled payments efforts at OHSU. 
  • Staff education around geriatric syndromes, care transitions, team communication, and others. 
  • Provide direct patient care at the Internal Medicine Clinic on Marquam Hill 2 half days per week.

 

The successful candidate must have demonstrated ability to work with a team of health care providers and collaborate to achieve clinical care goals. Our practice also includes innovative models for delivering structured care to populations of patients with chronic conditions. Candidates with an interest in and experience with chronic care models of health care, coordination of care of complex patients in various settings, and continuous practice improvement will receive priority consideration. Our practice is recognized by the State of Oregon as a Tier 4 Patient Centered Primary Care Home and a Comprehensive Primary Care Plus clinic site. Faculty in the Division of General Internal Medicine & Geriatrics participate actively in teaching medical students, residents, fellows, and inter-professional students.

 

Opportunities for teaching abound and participation is encouraged and valued. The faculty member will be expected to participate in continuing education, professional and faculty development and scholarship, as directed by his or her department. Compensation for this position will include a base academic salary, paid through the academic department, plus a clinical salary from an incentive system based on the success and productivity for their clinical work at OHSU campus, as well as personal initiative. 

Required Qualifications

  • BLS certification required
  • Demonstrated commitment to educational excellence is required
  • Minimum of 2 years of experience with a focus on care of older adults

Nurse Practitioner:

  • Current Nurse Practitioner license for the state of Oregon
  • MSN or DNP

Physician Assistant:

  • Current & Active Physician Assistant license for State of Oregon
  • MSN or Master's degree as a Physician Assistant
